Preferred Partner Membership Application Addendum

Instructions: Company membership includes one (1) Primary Membership and up to four (4) Secondary Memberships. The Primary Member, who is responsible for appointing Secondary Members to represent the Company and to notify SAR of any changes in membership or contact information, should be listed in the Contact Information. Secondary Members should be listed on the second section of this application. There is an additional charge for each Secondary Member above the first.

If your company has more than one office, please complete the Additional Office Information section below and list all Secondary Members assigned to that office. Complete one form for each additional branch or location.
